- if you meet the client use 90837, if you meet the partner use 90846, if you meet both use 90847
**Individual therapy**
- **90834** – 45 minutes psychotherapy
- This is the most common one
- **90837** – 60 minutes psychotherapy
- Use only when you truly go long. Audited more often.
- **90832** – 30 minutes psychotherapy
- Short sessions, check-ins, or constrained schedules.
**Family and couples**
- **90847** – Family or couples therapy with patient present
- THIS is the standard couples therapy code.
- **90846** – Family therapy **without patient present**
- Parent sessions, or meeting with the partner of a client in the couple.
**Group**
- **90853** – Group psychotherapy
## Intake and evaluation
- **90791** – Psych diagnostic evaluation
- No meds. Use for intakes. Bill once per episode of care.
## Telehealth modifiers and place of service
This is where people mess up.
- **Modifier 95** or **GT** depending on payer
Many insurers now accept 95.
- **Place of Service**
- **10** = Telehealth provided in patient’s home
- **02** = Telehealth not in patient’s home
Always check the payer. They are inconsistent.
## Diagnosis codes (ICD-10)
You still need one, even if it feels forced.
Common examples:
- **F41.1** – Generalized anxiety disorder
- **F33.1** – Major depressive disorder, recurrent, moderate
- **F43.23** – Adjustment disorder with anxiety and depressed mood
- **Z63.0** – Relationship distress with spouse or partner
Often used with 90847, but some insurers hate Z-codes.
